Need help getting to an appointment? Many Champion Health Plans cover approved transportation, when needed. Our Transportation Services Staff is ready to assist you.
We recommend you call at least 48 hours before your appointment, if possible. Have your Champion Health Plan member ID and appointment details ready.

How far ahead do I need to schedule a ride?
Call at least 48 hours before your appointment when possible. For an urgent, non-emergency need, call us right away.

Tell us when you schedule your ride. We will work to arrange the right type of transportation and support.
