Directory Information & Accuracy
We update provider information within 30 calendar days of receiving new information. The directory includes:
- Provider name
- Specialty
- Address
- Phone number
- Network participation status
- Whether the provider is accepting new patients
- Plan-specific participation details
The “Last Updated” date appears on all online, printed, and downloadable versions. You may:
- View providers online
- Download or print directory information
- Request a paper directory at no cost by contacting Member Services
Before you schedule
Provider participation may change at any time. Before scheduling an appointment, confirm with the provider’s office that they participate in your specific Champion Health Plan product.
Plan Types & Provider Requirements
HMO Plans
- You must choose a Primary Care Provider (PCP) from our network.
- Your PCP coordinates and oversees your care.
- Referrals may be required to see specialists.
HMO-POS Plans
(Champion Advantage, Champion Connect, Champion Select)
- You must select an in-network PCP.
- You must also select an in-network Nephrologist (Primary Treating Provider – PTP)
