Getting the care and support you need should not be hard. Your Championed Care Concierge Team helps you, your caregiver, and your doctors.*
Your Care Concierge Team can help you find a specialist, set up an appointment, and get seen faster when you are having trouble. They can also help keep your care connected between you, your caregiver, and your doctors.
Beyond medical care, they can connect you to community resources for food, housing, or utilities, help you apply for Medicare Extra Help, and help with your Medicaid or Medi-Cal renewal.
Who can use this benefit:
- All Champion Health Plan members, at no extra cost
- Members who need help finding a specialist or getting a timely appointment
- Members and caregivers who need help connecting to community resources or financial support programs
