Home Based Dialysis Support

As a Champion Health Plan member, we recognize that each person’s care needs to be individualized to help achieve the best outcome and approach. If receiving dialysis treatment at home is best for you, we have benefits to help you be successful. These added benefits include:

• Receive a visit from a trained caregiver to perform fistula or graft cannulation and machine setup at the beginning of home hemodialysis treatments*
• Respite services so your family member or caregiver and take time off for vacations or illness*
