Transportation That Helps You Get the Care You Need
Reliable transportation can make it easier to get the healthcare you need. If you do not drive or need help getting to an appointment, Champion Health Plan may provide transportation to and from approved healthcare locations.
Covered transportation is available with a $0 copay. The number of rides, distance, and other benefit details vary by plan. Check your Evidence of Coverage to learn what your plan includes.
Schedule a Ride
The Champion Health Plan Transportation Support Center is available 24 hours a day, 7 days a week to help with non-emergency medical transportation.
- Call at least 48 hours before your appointment whenever possible.
- For an urgent, non-emergency transportation need, call us right away.
- Have your Champion member ID card and appointment information ready.

Need Extra Help?
Tell us when you schedule your ride if you need special help, such as wheelchair assistance from your home to the vehicle or from the vehicle to your appointment. We will work to arrange the right type of transportation and support.
