At Champion Health Plan, we understand that reliable transportation is essential to accessing quality healthcare. That’s why we make it easier for our members who do not drive, or who need assistance, to get to and from their healthcare-related appointments.

Champion Health Plan provides a $0 copayment for transportation to approved locations. Benefits vary by benefit plan. Please see your Evidence of Coverage for exact benefits.

 Scheduling a Ride

• Please call Champion Health Plan at least 48 hours in advance to schedule transportation for approved healthcare-related appointments.

• For urgent, non-emergency medical transportation needs, call us right away.

Call Champion Health Plan at 1-800-885-8000 | TTY 711 to schedule transportation to approved healthcare providers, 8:00 a.m. to 8:00 p.m., 7 days a week (except Thanksgiving and Christmas) from October 1 through March 31, and Monday to Friday (except holidays) from April 1 through September 30.

Special Assistance

If you need additional help, such as wheelchair assistance from your home to the vehicle

and from the vehicle to your appointment, please let us know when scheduling your ride

so we can ensure the right support is available.
