What Is a C-SNP?

Chronic Special Needs Plan (C-SNP) are Medicare Advantage Plans that specialize in providing coverage to Medicare beneficiaries with certain medical issues, such as diabetes, congestive heart failure, cardiovascular disorders, and end stage renal disease to name a few.

Champion Health Plan is a C-SNP that specializes in help people with end stage renal disease better manage their medical conditions, find their optimal wellbeing, and save money on the cost of care.

With our Health Plans you are at the center of what we do. Together with your Nephrologist (Your Primary Treating Physician “PTP”) have healthcare resources to meet your needs. You will have direct access to your Plan participating Primary Treating Physician without the need for a referral.

You will still select a Primary Care Physician (PCP). Your PCP and PTP will become a team to help you manage your health care needs.

You will have options for Dialysis care. Our health plans cover the use of its Participating Dialysis Centers as well as use of non-participating or contract Dialysis Clinics at a lower copayment than original Medicare. If you choose to use a non-participating Dialysis Clinic, you will need to get prior authorization. We also have a Home Assist program for Dialysis treatments. If it is right for you, we will make arrangements for you to receive your dialysis treatments in the comfort of your home.
