What Is a C-SNP HMO-POS Plan?
Chronic Special Needs Plans (C-SNP) are Medicare Advantage Plans designed for beneficiaries with specific medical conditions, such as diabetes, congestive heart failure, cardiovascular disorders, and end-stage renal disease.
Champion Health Plan is an HMO-POS C-SNP focused on helping individuals with end-stage renal disease manage their health, improve their quality of life, and reduce healthcare costs.
HMO-POS (Health Maintenance Organization Point-of-Service) Plans offer the flexibility of both HMO and PPO plans, allowing you to see out-of-network physicians with prior authorization. However, you must still choose an in-network Primary Care Physician (PCP) and Nephrologist (Primary Treating Physician, PTP), who will work together to manage your healthcare needs.
