What Is a Medicare Advantage Prescription Drug Plan (MAPD)?
A Medicare Advantage Prescription Drug Plan (MAPD) combines the benefits of Medicare Part A (hospital insurance), Medicare Part B (medical insurance), and Medicare Part D (prescription drug coverage) into one convenient plan.
MAPDs typically offer extra benefits not included in Original Medicare—such as dental, vision, hearing, transportation, healthy food, and over-the-counter allowances—to support your overall health and well-being.

What Is a C-SNP HMO Plan?
A Chronic Condition Special Needs Plan (C-SNP) HMO is a Medicare Advantage Prescription Drug Plan tailored for people living with certain chronic health conditions.
Members select an in-network Primary Care Physician (PCP) who coordinates care, provides referrals to specialists, and ensures that treatment is personalized and condition-focused.
Champion Health Plan’s C-SNP HMO Programs
- Champion Care – Designed for members with Diabetes and or cardiovascular disease.
- Champion Choice – Designed for members with Diabetes and or cardiovascular disease. This plan also integrates seamlessly with Medicaid (Medi-Cal in California) to improve care coordination and deliver enhanced benefits for dual-eligible members.
- Champion Plus – Designed for members with serious behavioral health conditions, including Schizophrenia, Schizoaffective Disorder, Bipolar Disorder, Major Depression, and Paranoid Disorders. Members have direct access to a psychiatrist who works in close coordination with the member’s Champion PCP to ensure comprehensive mental and physical health support. This plan also integrates with Medicaid (Medi-Cal) for added services and care coordination.
What Is a C-SNP HMO-POS Plan?
A Chronic Condition Special Needs Plan (C-SNP) HMO-POS combines the coordinated care of an HMO with the flexibility of a Point-of-Service (POS) option.
Members choose an in-network PCP and a Primary Treating Physician (PTP) – typically a Nephrologist—to lead their care team. While most services are provided in-network, members can also access approved out-of-network providers with prior authorization.

- Champion Advantage, Champion Choice, and Champion Select – Tailored for individuals with chronic kidney disease (CKD), including end-stage renal disease (ESRD).
